 The transition from school for individuals with disabilities can be daunting and confusing. Adult life amplifies and adds to these challenges. We have many years of experience helping individuals and families navigate through the complexities of resources for transition, benefits and adult services in Maryland to achieve success. We understand the components of these processes that lead to a successful future for adults with disabilities and focus that knowledge on your individual concerns and personal situation.

Our Approach

We provide a comprehensive, customized and supportive approach to meeting the needs and preferences of individuals and their families. We help clients develop an understanding of and apply for resources and services, and assist with transition plans, oversight, resource navigation and managing and maximizing adult services and success.  We provide personalized transition and adult services counseling and strategies to achieve the long term goals and meet the needs of individuals and families as they continually evolve over a lifetime.

Our Services

Entitlement Programs

Identify, apply for and maximize Public Benefits and Funding Sources such as Social Security, DDA, DORS, Medicaid, Medicare, SNAP and Housing Assistance.

Person Centered Plans

Develop goals, maximize utilization of DDA services, assist with plan or service provider changes.

Service Providers

Assist in evaluating, selecting, coordinating, overseeing and managing new or existing services.

Transition Plans

Prepare for post-school and adult life and assist in developing life skills and in-home supports.

Education

Assist in identifying community resources and in gaining a full understanding of adult services particular to your concerns.

DDA Service Models

Help you to understand, choose between and manage Self Directed and Traditional service models.

Continuity Planning

Collaborate with family members, Guardians, Trustees, Life Plans and Letters of Intent to provide continuity, oversight of services and quality of life.

Employment Services

Assist in career exploration and counseling, and provide hands-on training to help clients prepare for work, get hired, and maintain their employment.

About Us

Disability Transition Support was founded by Tom Liniak in 2018 to prepare and assist an ever-growing number of families and individuals who need a practical and comprehensive resource for success after leaving the school system. Services are available to address the changing needs that arise throughout the different stages of adult life.

Tom is a recognized expert and innovator in the fields of disability employment  and disability services with over 20 years of experience. Across his career, he has led programs that have delivered record results for job candidates at two Maryland DDA providers and over 1,200 job placements. As the Director of Employment Services at the Jewish Social Services Agency (JSSA), he led the agency to Maryland Disability Employment Provider of the Year recognition. He also served as the Director of Community Based and Workforce Services for the Maryland Division of Rehabilitation Services (DORS). At DORS, he led statewide employment and skills training, job placement assistance, and workforce and rehabilitation technology services. He was also appointed to DORS Executive Staff to plan, develop and manage DORS statewide policy and operations. Tom has served on the Maryland State Rehabilitation Council, State of Maryland Employment First Committee, the Montgomery County Commission on People with Disabilities and as a DDA Certified Support Broker.

He is also a licensed attorney and the co-founder of the award winning nonprofit Sports Plus, whose innovative swimming, sports and social programs for those with disabilities have been studied by the University of Colorado. Tom also brings the personal perspective of a parent of a young adult with autism to all of his work with families.
